The State or County Health Officer grants the waiver on this written evidence. The Department of Health's Religious Waiver Request may be used instead.
The paper. Religious Waiver Request (form IMM-003), Wyoming Department of Health Immunization Unit, or a signed parent statement. The state publishes the form. How long it lasts: No end date or renewal is stated in the law, the state rules, the health department page or the form. A waiver does not cover a shot added to the list later. That needs a new request.
