   Keep.

   Shot records. Or the doctor's letter or exemption letter. This is what you attach to the form.

   Where.

   A folder at home. Immunizations

3. 3. Teaching log

   Keep.

   A log you fill in as you teach. List the reading materials by title. Show the 180 days, or 900 hours (grades K to 6) or 990 hours (grades 7 to 12).
