28 Pa. Code 23.84 offers those two grounds. Keep the one that is true and strike the other. It attaches to the home education affidavit.
The paper. A written objection signed by the parent, the guardian, or a child who is legally on their own. There is no state form to fill in. How long it lasts: Attached to the homeschool form each year.
