That is choices 2 to 5.The law asks for proof of shots with your first Form A.The state's health office says only two groups must send it:A child taught under a licensed teacher, or in a district program. That is choices 4 and 5.A child in dual enrollment, which means also taking public school classes or activities.A child on choice 2 with no dual enrollment sends nothing.Choice 1, Independent Private Instruction, is exempt, the health office says.
