Personal belief exemption

Step 1: Get the paper. State conscience exemption form F11-11755 (a signed form called an "affidavit"). Open the form
Step 2: Who signs it. The state publishes the form. A notary has to witness it. 1.Print form F11-11755 from the Texas health department website.2.Sign it in front of a notary.3.Give it to the school or child care within 90 days.
Step 3: How long it lasts. It has to be written again every two years.
