2. Step 2: Who signs it. The state publishes the form. 1.Print the medical exemption form.2.Have the doctor sign it.3.Give it to the school nurse to fill in the school part.
3. Step 3: How long it lasts. How long it lasts: The form and the rule do not say it must be renewed.
4. Step 4: Put it in the folder. Keep the records
