Religious exemption

1. Step 1: Get the paper. Religious Exemption Statement, form HES 113. Open the form
2. Step 2: Who signs it. The state publishes the form. 1.Print the schools version for a school, preschool or college.2.Print the child care version for child care.3.Sign it, date it and hand it in.
3. Step 3: How long it lasts. It does not expire.
4. Step 4: Put it in the folder. Keep the records
