Religious exemption statement

District of Columbia accepts a statement written at home. Fill in the blanks, print it, sign it.

It goes to DC Health by way of the school or licensed child care facility, and DC Health's own certificate is good for one school year, July 1 through June 30.

The paper. Annual Immunization Religious Exemption Certificate, or another writing the parent or adult student signs. The state publishes the form. It has to be written again every year.
